Love in the Time of Corona

Tehran (ISNA) - This is not a doctor and this is not an angle, but the both are bloomed in the eye of a person (Picture 1). Both become one. As new hope comes, the dark clouds would go away. This painting says more regarding the burden of disease, burden of fear, and burden of people in our country; Iran. 

Imagine a respiratory referral hospital which is determined as an isolated center, with crowed of people in emergency section. Among the persons whom stocked in line, suddenly one arrived with his companies; a 56-year famous ENT specialist who admitted from another hospital, referred with positive COVID-19, directly in our critical care unit. His situation was in moderate ARDS and the O2 sat was 85%; CXR showed bilateral infiltration. Intermittent non-invasive ventilation was started; nasal High flow therapy; also, reservoir mask. His wife referred in better situation and sent to the floor for 2 days. And they are not just the only persons routinely referred to the hospital due to COVID-19; an outbreak of novel coronavirus which first began in Wuhan, China, then has spread rapidly, with cases now confirmed in multiple countries including ours. Patients in all range of age, the older ones are at higher risk of death, however it is not totally OK for younger ones too. Unfortunately, due to lack of news, lots of ENT specialist’s patients and friends thought he is dead due to coronavirus and sending memorials and obit schedules for him.

A 25-year-old physician who admitted from another hospital, referred here with diagnosis of H1NI and moderate ARDS. But second H1N1 test was negative. His situation was in moderate ARDS and the O2 saturation was 80%; CXR showed bilateral infiltration. After 5 days that the patient was admitted to the ICU ward, the coronavirus detection kit arrived in Iran; his test result was positive. Although his condition is not generally bad, you can see the fear in his dark eyes, the fear which invisible not only on his face, but also all around. The panic could be seen in streets, in closed markets and bazars, in limited public cultural and artistic programs. Fear creates fear. Panic surrounds us in Tehran metropolitan, and then in all cities in country; from home to home and from household to household. Isolating in home for the people who are known for closed family relationships, produced upset and emotional tension and stress. And high stress could disrupt the immune system! New Iranian year (Nowruz) is coming and nobody is happy of this traditional celebration!

For people who get used to live with sanctions, lack of facilities and resources, political tensions and finally the shadow of war, COVID-19 could be out of their tolerance. Each day we are facing with a large group of people who are afraid of this scary disease or getting hypochondriasis because of a small sneeze! Working in coronavirus ward getting hard and harder for clinic staff. The number of infected doctors and nurses are increasing. There is not enough masks and gloves. Access to medicines and medical facilities is limited due to US sanctions. Hope could vanish behind the tears and fears.

We tried to send some hopeful news; first, about our famous friend; the ENT specialist. Just making a short clip of him and introduce his improving conditions. The film was spread through the social media, maybe faster that COVID-19 epidemic! And getting positive feedback. Besides, there are some others who tried to share their experience with COVID-19 as a patient or medical doctor or specialist using social media and their cellphones. A new epidemic comes; the epidemic of podcast, short film, arts, and clips. Social media becomes the new weapon against COVID-19. Along with showing patients' videos clip through the cyberspace, patients' fears of disease have diminished. The feeling of solidarity and love is strengthened. Humanity connects people like a chain of unity. 

We did a lot of effort to prevent the young infected physician from intubation; through using noninvasive mechanical ventilation, high flow nasal cannula plus corona protocol, fortunately he was discharged from intensive care unit after undergoing critical conditions and he went into the non-critical care unit and finally, he was discharged from the hospital. At the time of leave, he filmed with his cellphone a short clip of himself and his doctor, expressed his gratitude. Now you can see the hope, happiness, and love. The life was shining in his dark eyes. This is not a reflection of a doctor or an angel, or even affection between doctors and patients.  This is hope and love which borne again in eyes of a nation, love at the time of corona…
